 How To Use These Forms

To use these forms, please download, print and complete these forms, and then submit them in person with any applicable fees to the Doshinkan office.

If you have been given permission to mail these forms to the Doshinkan Office, please include any applicable fees along with the forms, in the form of a check, or make sure to pay the applicable fees through PayPal before sending the form. The Dojo address is:
